The Anticoagulation Department is a Nurse-led service responsible for the safe anticoagulation monitoring and management of patients within the borough of Walsall. The service provides initiation of anticoagulation. education and counselling of patients on anticoagulation, INR monitoring and dose adjustment for patients on Vitamin K antagonists and initial review of patients initiated on Direct Oral Anticoagulants (DOACs).
The Anticoagulation Team is based in Route 020 Pathology at Walsall Manor Hospital. We run anticoagulation clinics five days a week in the community at different sites across the borough.
Referrals can be made directly to the service via email by a healthcare professional. Please note we do not make decisions on length of treatment. This should be decided by the clinician making the decision to start anticoagulation.
A referral form can be requested from the team to ensure all necessary information is provided.
