Community Nursing


What We Do

The Community Nursing Service provides comprehensive care to all residents of Walsall who are registered with a Walsall GP. Our service offers a wide range of support, including co-ordinated care and case management for individuals with long-term conditions. We deliver planned nursing interventions and supportive care, working closely with other agencies to help patients achieve maximum independence. We also provide advice and information on various health issues, ensuring patients have the knowledge they need to manage their health effectively.


Where We Are Based

Our services are delivered directly to patients, predominantly in their own homes. For patients who are not housebound, we will visit them at home if their nursing needs are best met there. We also provide specialist care, such as wound care, in local clinics to ensure patients have access to the best possible facilities. Our clinics and teams are based across four main locality centres:


How to Access the Service

You can access the Community Nursing Service through self-referral, or via a referral from your GP or another health professional. Referrals can be made by email or telephone.

Hours of Operation: The service operates 24 hours a day, 7 days a week.
