Frail Elderly Pathway (FEP)


What We Do

The Frail Elderly Pathway (FEP) team at Walsall Healthcare NHS Trust works directly within the hospital’s Emergency Department (A&E). The main goal of our service is to prevent unnecessary hospital admissions and ensure a safe and timely discharge for patients. After a thorough assessment, we facilitate a safe discharge by providing a range of services, including:

  • Providing equipment: We can provide low-level equipment, such as commodes and walking frames, to help patients remain independent at home.
  • Arranging care packages: We can rapidly set up a care package to meet the patient’s immediate needs.
  • Referrals and follow-ups: We can arrange follow-up appointments with community nurses, physiotherapists, occupational therapists, or social services.
  • Liaison: We work closely with a wide range of multidisciplinary teams to ensure a coordinated and effective care plan.

Our team also provides a follow-up phone call the day after discharge to check on the patient’s well-being.

How to Access the Service

This is not a public-facing service. The FEP team works directly with healthcare professionals in the hospital’s Emergency Department (A&E). Patients are referred to us by the A&E staff to help facilitate their discharge.
