Intermediate Care Service (ICS)

Our Intermediate Care Service (ICS) is a health and social care integrated service which supports patients at home or in a short stay care home bed, following a period of crisis or ill health.

It is based across the hospital and community, and includes a broad multidisciplinary team including Physiotherapists, Occupational Therapists, Nurses, Social Workers, Reablement Officers and other health and social care workers.

Our service covers three main Pathways (Intermediate Care Framework – NHSE 2023):

  • Pathway 1 – Reablement.
  • Pathway 2 – Bed-Based Rehabilitation / Short stay interim placement.
  • Pathway 3 – Interim placement for long-term care and support assessments

The aim of the service is to minimise long-term care needs and maximise independence and quality of life, supporting people to remain in their own homes.

How to access the service


ICS Therapy – A completed referral form needs to come from either a health or social care professional, or via the hospital discharge team to the ICS Therapy referral email: community.icstherapy@nhs.net.
