Stroke Community Services (Stafford)
What We Do
The Stafford Community Stroke and Early Supported Discharge (ESD) Team provides specialist support for people in the Staffordshire area who have had a stroke. Our team of stroke specialist nurses, occupational therapists, physiotherapists, and speech and language therapists helps you continue your rehabilitation at home. This allows you to receive the same intensive, expert care you would in the hospital, but in a familiar and comfortable setting.
We create a personalised rehabilitation plan with goals we agree on together to help you reach your full potential. Our support can last for up to two years after you leave the hospital.
We also offer various support and rehabilitation groups at our clinic. These groups help with communication, mobility, and managing emotional well-being, fatigue, and anxiety. They also provide a great opportunity to connect with other stroke survivors.
Where We Are Based
Our team primarily works with patients in their homes. We also hold individual and group therapy sessions at our clinic.

Our service operates from 8:30 am to 4:30 pm, Monday to Friday. Outside of these hours, you can leave a message on our answering machine.
How to Access the Service
Our service is for people who have had a stroke and are registered with a Stafford GP. The decision to offer Early Supported Discharge (ESD) is made by a hospital stroke team in discussion with you and your family.
We only accept referrals from health or social care professionals, which must be sent via email or letter.
