We are an ESD (early supported discharge) and a Community Stroke Team (CST) made up of stroke specialist Nurses, Occupational Therapists, Physiotherapists, Speech and Language Therapists and Assistant Practitioners. The team provides support for people who have had a stroke within Stafford and surrounding areas and who are under the care of a Stafford GP. The MDT team supports people for up to two years post stroke following discharge from hospital in various community settings. We support individual rehabilitation programmes based upon agreed goals and support people to reach their maximum potential. Various support and rehabilitation groups are also offered in clinic if clinically indicated.
