The Community Children’s Nurses (CCN) provide a dedicated Nursing service for sick children who have a Nursing or palliative care need.
The aim is to support the holistic health, educational and social care needs of sick children by providing Nursing care in the community setting, empowering and enabling the child, or young person and their family/carers to become more competent in the management of their condition, thereby reducing the need for hospital admissions, enabling early discharge and providing end of life care.
Where we are based
The CCN team is based in an office on the Children’s Ward at Walsall Manor Hospital.
Ward 21 is based on the second floor of the hospital, on Route 221. To protect our patients, all patients and staff need to buzz in and out of the unit using the intercom.
How to access the service
To access the CCN service, a referral will be made from a healthcare professional. This may be from a GP, hospital Consultant, ward staff, or other healthcare professionals who feel you would benefit from the service. To be referred, your child must have a Nursing need such as a nasogastric tube, gastrostomies, home oxygen, regular injections, wound dressings, central line care, port care, intravenous antibiotics.
