Walsall Healthcare NHS Trust is dedicated to providing the best possible quality of life for patients experiencing life-limiting conditions. Our Palliative Care service offers expert assessment, advice, treatment, and support for a range of symptoms and issues. We work closely with other agencies to ensure our information is current and our care is coordinated.
Our service is made up of an integrated team of hospital-based and community-based specialists. We see patients who have moderate to complex needs, and our support can range from a single visit to ongoing care, whether you’re in the hospital or at home.
Our team includes:
The team’s goal is to maintain the best quality of life for both patients and their carers. We offer advice on a number of issues, including symptom control, emotional and financial support, and providing clear information.
Our service operates from two main locations to provide comprehensive care.
Community Team The Community Palliative Care team is based at the Walsall Palliative Care Centre and visits patients in their homes throughout the Walsall Metropolitan Borough.
Hospital Team The hospital-based team covers all wards and departments within Walsall Manor Hospital.
Our teams can be reached from Monday to Friday between 9am and 5pm. If you call outside of these hours, you can leave a message on our answer machine.
Referrals to the Palliative Care service must be completed by a healthcare professional who is already involved in your care.
We understand that dealing with life-limiting conditions can be very emotional for families. We ask that you please treat our team members with the same respect that you would want for yourself.
