We provide an integrated service in Walsall covering the community, hospice, day hospice and acute settings. We provide Specialist Palliative Care Therapies to patients with limited prognosis, promoting quality of life and empowering independence.
This includes crisis intervention to prevent hospital admission, facilitating preferred place of care and death through rapid discharge planning and/or urgent work within the community setting. It also includes complex emotional support as we work with individuals to accept their ongoing functional decline as their disease progresses, Advance Care Plans, palliative rehabilitation, and encouraging living well with limited functional ability.
Where we are based
Our integrated service covers both acute and Community settings – we cover Hospital at Walsall Manor Hospital, Inpatients at Walsall Hospice, Fair Oaks Day Hospice and Walsall Community Services.
How to access the service
To access the service, we accept referrals from other healthcare professionals and GPs. We have referral forms which can be emailed to us.
