The Outreach Support Service supports individuals with a new or existing cancer, palliative diagnosis, or life-limited illness. The team provides emotional and practical support from diagnosis through treatment and beyond, including for carers and families. Its primary aim is to ensure patients receive the right information, care, and support tailored to their needs. The team helps navigate services, co-ordinate care, and promotes informed decision making at any stage of the illness.
The Information Centre complements this by offering accessible, trusted health information and resources for the community, community services and professionals.
