Community Clinical Nurse Specialists and Specialist Consultants support patients with a life-limiting or palliative diagnosis, offering holistic assessment and treatment for all symptoms including physical, psychological, and social aspects of care. We work throughout the borough of Walsall visiting patients with moderate to complex needs in their own homes or care setting in the community.
Where we are based
We visit patients in their own homes or community care setting within Walsall and work from a base at Walsall Palliative Care Centre a few miles outside of the town centre.
How to access the service
Referrals must be made from a healthcare professional, this could be a GP, Community Nurse, or hospital team.
Specialist Nurses triage all referrals to complete an initial assessment of the palliative care needs.
