Community Matrons provide holistic care to housebound patients with complex, long-term conditions. Our Nurse-led service focuses on clinical, social, and psychological support, including expert medicines management. We aim to prevent unnecessary hospital admissions by creating preventative care plans and proactively managing chronic diseases within the community. If a patient is admitted to hospital, we work closely with the hospital staff to reduce the length of their stay and ensure a smooth, timely discharge to the most suitable setting.
Our Community Matrons are embedded within the seven district nursing teams across Walsall to ensure localised and responsive care. The service is managed from the following address:
