Self Care Management Team

What We Do

Our Self-Care Management Service helps people with long-term health conditions and their carers better manage their health on a day-to-day basis. Our programmes are designed to give you the skills, confidence, and information you need to make informed decisions about your care and live a healthier life. All of our courses are completely free and run for 2.5 hours, once a week, for six weeks.

We offer several specialised programmes:

  • Self-Care Management Programme for Long-Term Health Conditions: For adults with conditions like heart disease, lung conditions, or arthritis. Topics include getting better sleep, managing emotions, and communicating with healthcare professionals.
  • Diabetes Self-Management Programme: Specifically for people living with diabetes, covering topics like healthy eating, monitoring blood glucose, and preventing complications.
  • Chronic Pain Self-Management Programme: Helps you learn to manage chronic pain with techniques such as keeping a pain diary and gentle exercise.
  • “Caring for Me and You” Course for Carers: This course is for adults who care for someone with a long-term health condition or disability. It focuses on helping you look after your own health and wellbeing while you’re caring for someone else.

By taking part, you’ll gain knowledge, feel more in control, and meet others who share similar experiences.
