Stroke Community Services (Cannock)
What We Do
The Cannock Community Stroke and Early Supported Discharge (ESD) Team provides specialist support for people in the Cannock area who have had a stroke. Our goal is to help you continue your recovery at home with the same level of care you’d get in hospital.
Our multidisciplinary team includes:
- Stroke Specialist Nurses
- Occupational Therapists
- Physiotherapists
- Speech and Language Therapists
We provide a personalised rehabilitation plan tailored to your specific needs and goals, helping you to reach your maximum potential. We offer support for up to two years after you’ve been discharged from hospital.
We also offer various support and rehabilitation groups at Springfields Health and Wellbeing Centre, including sessions for communication, mobility, and managing emotional well-being and fatigue.
Where We Are Based
Our team works with patients in their own homes. We also hold individual and group therapy sessions at our clinic.

We are open from 8:30 am to 4:30 pm, Monday to Friday. Outside of these hours, you can leave a message on our answering machine.
How to Access the Service
We accept referrals for people who have had a stroke and are registered with a Cannock GP. The decision to offer Early Supported Discharge (ESD) is made by a hospital stroke team, in discussion with you and your family.
You have lifelong access to our service, but any new referrals must be for stroke-specific needs. All referrals must be made by a health or social care professional via email or letter.
