Your Cancer Care Navigator


What we do

Your Cancer Care Navigator works with the Clinical Nurse Specialist team to provide ongoing support throughout your cancer pathway. A Navigator will contact you to arrange a phone consultation for a holistic needs assessment. The assessment is an opportunity to discuss any concerns or worries you may have, it’s a supportive consultation to help identify any ongoing needs and provides time and opportunity for you to identify any emotional, practical, physical, spiritual or financial concerns. After the assessment the cancer care navigator will let you know, what support is available to manage your concerns, they may refer you to other services such as a benefits team, or a local support group to help you manage what is important to you. Either during or at the end of your treatment the cancer care navigator will offer you the opportunity to attend a health and well-being event. This help provides ongoing support for you and your family as well as the opportunity to meet and share experiences with other cancer patients.
